Practice Areas: Family Practice, Pediatrics, Internal Medicine, OB/GYN and Psychiatry

Specialists: Yes. For specialists practicing in a non-designated area, the facility must document 30% of the total number of specialty patients in the past year can be from surrounding designated areas or meet the “Flex Placement Criteria” listed under the Wisconsin Conrad 30 Program – Program Description.

Application Review: Applications are accepted October 1 and evaluated on a first-come, first-served basis until all 30 slots are filled.

Notes: Full-time is defined as 40 hours primary/medical care per week, with at least 32 hours in direct patient care. Signed Applicant Physician and Employer Assurances sheets are required. Applications for Flex 10 spots will not be considered on a first-come, first-served basis, and based on need. New Flex 10 guidelines effective October 1, 2016.
